תסמונת גירוי יתר שחלתי (OHSS, Ovarian hyperstimulation syndrome) היא תגובה יתרה לגירוי שחלתי, הכוללת הגדלת שחלות, צבירת נוזלים בחללי גוף ועלייה בצמיגות הדם העלולה לגרום לסיבוכים עד כדי כשל איברים חיוניים ותמותה.

פתופיזיולוגיה

התסמונת המתאפיינת בדליפת נוזלים (Capillary leak syndrome) מסיבית מהמערכת התוך-כילית לחללים החוץ-כיליים (Third space).

גורמי סיכון

  • גיל צעיר, משקל גוף נמוך, היארעות OHSS בעבר, תסמונת השחלות הפוליציסטיות - PCOS, רמה גבוהה של AMH ומספר גבוה של זקיקים אנטרליים (AFC)
  • תגובה שחלתית מוגזמת במהלך טיפול:
    1. בטיפולי השראת ביוץ/ביוץ יתר - התפתחות של יותר מ-4 זקיקים בקוטר 14 מ"מ ומעלה, ערכי אסטרדיול גבוהים מ-pmol/L {{כ}5500‏ (pg/ml ‏1500)
    2. בהפריה חוץ-גופית - שימוש בפרוטוקול GnRH Agonist ארוך, או התפתחות של יותר מ-25 זקיקים בקוטר 14 מ"מ ומעלה, ערכי אסטרדיול גבוהים מ-pmol/L ‏13000 ‏(pg/ml ‏3500, או שאיבה של יותר מ-24 ביציות
  • חשיפה ל-hCG במהלך הטיפול או בהיריון כתוצאה מהטיפול

דרכים להפחתת הסיכון ל-OHSS

תסמונת גירוי יתר שחלתי עלולה להופיע גם במצבים שאינם בגדר "תגובה שחלתית מוגזמת" לכן, אינה ניתנת למניעה באופן מוחלט. ניתן לצמצם את היארעותה וחומרתה על ידי שימוש בחלק או צירוף של האמצעים הבאים[2]:

  • זיהוי נשים עם גורמי הסיכון (סעיף 3)
  • גירוי שחלתי מתון, מבוקר ומותאם אישית[3]

בטיפולי פריון שאינם IVF

בטיפולי השראת ביוץ או גירוי שחלתי, עם זיהוי תגובה מוגזמת (סעיף 3 ב' 1):

  • עצירת הטיפול (להימנע ממתן hCG, לא לבצע הזרעה תוך - רחמית ולהורות למטופלת להימנע מקיום יחסי מין)
  • מתן אגוניסט לביוץ (עם תמיכה לוטאלית), עם מתן הסבר לסיכוי ולסיכון של היריון מרובה עוברים
  • מעבר להפריה חוץ-גופית באותו מחזור - Rescue/Emergency Conversion IVF

בהפריה חוץ-גופית (ע"פ סדר כרונולוגי של הפעולות במהלך הטיפול)

  • מתן Metformin בנשים עם PCOS ‏(Grade A) ‏[4][5][6][7][8]
  • שימוש בפרוטוקול: GnRH Antagonist ‏(Grade A), ראו נספח‏[9][10][11][12][13][14]
  • coasting (הפסקת גונדוטרופינים ומתן Trigger hCG) - אין ראיות מספקות (Grade C)‏[15][16]
  • הפסקת מחזור הטיפול
  • שימוש ב- Trigger GnRH Agonist במקום ב-hCG במקרים המתאימים (Grade A)‏[17][18][19][20][21][22][23][24][25]
  • הפחתת מינון Trigger hCG - אין ראיות מספקות (Grade C)‏[26]
  • מתן אגוניסט לדופאמין החל מיום מתן ה-hCG למשך מספר ימים (Grade A)‏[27][28][29][30][31][32][33][34][35]
  • מתן אלבומין בעת השאיבה - אין ראיות מספקות (Grade C)‏[36][37]
  • הימנעות מהחזרת עוברים טריים, הקפאתם והחזרתם במחזור אחר (Grade B)‏[38][39][40]

טיפול

הטיפול ב-OHSS מבוסס על ניטור קפדני וצמצום סיבוכי התסמונת. בדרגות חומרה קלה ובינונית, יש להמליץ על מנוחה, שתייה, ומעקב אחר מתן שתן.

יש לשקול אשפוז כאשר מופיע אחד או יותר מהמימצאים הבאים[41]:

  • כאבי בטן חזקים או סימני גירוי ציפקי
  • בחילות והקאות משמעותיות המונעות אכילה ושתייה
  • אוליגוריה או אנאוריה
  • Tense Ascites
  • קוצר נשימה
  • ירידה בלחץ דם, סחרחורת או עילפון
  • המטוקריט מעל 45%
  • ספירת טסיות מעל 450,000 וספירה לבנה מעל 15,000
  • רמת נתרן בדם מתחת 7-135mEq/l
  • רמת אשלגן בדם מעל 5mEq/l
  • הפרעה בתפקודי כבד ו/או כליה
  • הפרעה בתפקודי קרישה

מעקב במהלך האשפוז

  • סימנים חיוניים כל 2–8 שעות בהתאם למצב הקליני
  • שקילה יומית
  • מאזן נוזלים (fluid intake and output)
  • הערכה סונוגרפית של גודל השחלות והימצאות מיימת וחומרתה
  • הדמיית בית החזה (צילום רנטגן או אולטראסאונד)
  • ניטור ריוויון החמצן בדם לפי הצורך
  • מעקב מעבדתי ע"פ הצורך (אלקטרוליטים, המטוקריט, ספירת טסיות, קריאטינין, תפקודי כבד ותפקודי קרישה)

טיפול במהלך האשפוז

  • העמסת נוזלים מהירה של 500-1,000 מ"ל
  • לאחר מכן, מתן נוזלים בכמות שתגרום למתן שתן בנפח של יותר מ-0.5 מ"ל/ק"ג משקל גוף/שעה
  • דקסטרוז 5% בסליין עדיף על תמיסת רינגר בגלל הנטייה להיפונתרמיה והיפרקלמיה
  • אלבומין (25%) 50-100 גרם, ניתן במשך 4 שעות וניתן לחזור כל 4 עד 12 שעות
  • פרצנטזיס ליברלי בכל המצבים בהם ניראה צורך קליני לכך (כגון בטן מתוחה, קוצר נשימה, אוליגוריה)
  • בנוכחות היפרקלמיה יש לנטר ולטפל בהתאם
  • יש לשקול מתן קלקסן מניעתי (0.5 מ"ג / ק"ג)
  • במקרי גירוי יתר שחלתי מסכן חיים בהיריון יש לשקול הפסקת היריון
