מבוא (מבואות 1-14)

הסתיידות העורקים הכליליים (CORONARY ARTERY CALCIUM (CAC הוא מדד המשקף את סך הטרשת הכלילית. CAC בנבדקים ללא מחלה כלילית קלינית מצביע על מחלה תת-קלינית וכמות ההסתיידויות משקפת את מידת חומרתה בדפנות העורקים. ניתן לאבחן CAC בכל סריקת CT חזה במינון קרינה נמוך הנעשית בהתוויות קליניות שונות ולכמתן בעזרת תכנה ייעודית או בהערכה וויזואלית. כמות ההסתיידויות מבוטאת בערך הנקרא (TOTAL CORONARY CALCIUM SCORE (CCS הנמדד ביחידות אגטסטון. שילוב מדד זה עם גורמי הסיכון הטרשתיים המקובלים מאפשר הערכת סיכון אינדיווידואלית באמצעות ה MESA RISK SCORE. הכללת ה- CCS בהערכת הסיכון מביאה לקביעה מחדש (re categorization) של הערכת סיכון בעד כמחצית מהנבדקים לעומת מדדי הסיכון המקובלים. הבדיקה מאפשרת למקד טוב יותר טיפול תרופתי אינטנסיבי למניעה ראשונית, לבחור נכונה את אלו שאכן זקוקים לו, ובהיעדר CAC, לצמצמו ואף למונעו, כפי שבא לידי ביטוי בהנחיות האחרונות לטיפול בסטטינים מטעם ה ACC, ה ,AHA, ה ESC ואיגודים רפואיים נוספים.

נייר עמדה זה מיועד לקרדיולוגים, פנימאים ורופאי משפחה העוסקים במניעה ראשונית של מחלת לב כלילית. יובא בקצרה הידע הרב שנרכש ב 30 שנות מחקר על משמעות ההסתיידויות, שיטות המדידה, ערכן בהערכת סיכון בנבדקים אי-תסמינים, יינתנו המלצות לשימוש קליני מושכל ב CCS תוך מתן מבואות (בכותרת לכל פרק משנה) המסכמות מחקרים, ניירות עמדה וסקירות שיאפשרו קריאה והבנה מעמיקה יותר.

הסתיידויות כליליות ומשמעותן (מבואות 15-29)

CAC הוא סמן חד-משמעי לטרשת באינטימה של דופן העורקים הכליליים. כמות ההסתיידויות קשורה בנוכחות פי 5 של טרשת בלתי מסוידת בעץ הכלילי. פלאקים רכים אלו )"טרשת רכה"( בעלי פעילות ביולוגית הנוטים יותר לתהליכי דלקת וקרע המאפיינים את התסמונת הכלילית החדה. פלאקים טרשתיים רכים מאפיינים את שלביו הראשונים של התהליך הטרשתי, שכיחים יותר בנשים, בצעירים ובאלה עם תסמונת כלילית חדה. מעקב אולטרסאונד תוך כלילי מצביע, שסטטינים פועלים בעיקר על פלאקים רכים אלו ולא על הפלאקים מסוידים.

ההסתיידויות הכליליות הן תוצאה של מכלול תהליכים ביולוגים מורכבים המושפעים ממספר רב של גורמי סיכון (וגורמים מגנים), גנטים וסביבתיים. שכיחות וכמות ההסתיידויות עולה עם הגיל ומואצת בגברים מעל גיל 50 שנה ובנשים מעל גיל 60. בצעירים מתחת לגיל 40 ובאלה המסתמנים לראשונה עם אירוע כלילי חד כמות ההסתיידויות מעטה יותר ונקודתית, בעוד שבמבוגרים ובאלה עם מחלה כלילית כרונית ממושכת, קיימת כמות מרובה של הסתיידות דיפוזית. ההסתיידויות מופיעות בשלביה המתקדמים של הטרשת STARY STAGES ‏(V-VII) ומהוות, ככל הנראה, חלק מתהליך ריפוי הרובד הטרשתי הבלתי יציב. תמיכה לתפקיד ההסתיידויות ביצוב הפלאק הטרשתי מובאת בסדרת מאמרים שפורסמו, בהן נבדקה השפעת הסטטינים על התקדמות הפלאק הטרשתי לאורך שנים בבדיקות אולטרסאונד תוך קורונריות של חולים עם coronary artery disease ‏((CAD במהלך צינתור. בעבודות אלה הוכח, שטיפול במינון גבוה של סטטינים לעומת מינון סטנדרטי, גרם לעליה משמעותית בכמות ההסתיידות בפלאק הטרשתי ולייצובו והביאה לירידה באירועים קרדיו-וסקולרים חדים. תצפית זו אושרה גם בעבודה שפורסמה, שבדקה בעזרת optical coherence tomography את הרכב הרובדים הטרשתיים בחולים עם אוטם חד, תעוקה בלתי יציבה ותעוקה יציבה. נמצא כי שטח ההסתיידויות ואורכן לאורך הכלים היה קטן יותר, עם מספר רב יותר של הסתיידויות נקודתיות באלה עם אוטם חד לעומת אלה שהסתמנו כתעוקה יציבה. עוד מצאו החוקרים כי ה-CULPRIT PLAQUE באוטם חד ובתעוקה בלתי יציבה היה שיטחי יותר עם שכיחות גבוהה של הסתיידות נקודתית לעומת הפלאקים בתעוקה יציבה. תצפיות אלה מאוששות עבודות דומות שהראו בעקביות, כי הסתיידות מרובה ודיפוזית מאפיינת מחלה כלילית כרונית יציבה, בעוד באלו עם תסמונת כלילית חדה נצפית כמות קטנה של הסתיידויות כשמרביתן נקודתיות. הוכחה נוספת לכך כי הטרשת המסוידת המרובה מיצבת את הפלאק הטרשתי ניתן למצוא במספר מחקרים, שהראו כי ההסתמנות הקלינית הראשונה בחולים עם הסתיידות דיפוזית מרובה, היא בעיקר תעוקת מאמץ ו/או מבחני מאמץ חיוביים ופחות אירועים כלילים חדים.

מהידוע, ניתן לסכם, שההסתיידויות מופיעות בשלבי הטרשת המתקדמים. כמות מרובה מאפיינת חולים עם מחלה כלילית כרונית אך ניתן לראותה גם בנבדקים אי-תסמיניים. כמות ההסתיידויות משקפת נוכחות של פי 5 פלאקים טרשתיים בלתי מסוידים המשמשים קרקע לאירועים כליליים חדים.

התרומה הפרוגנוסטית של ה- CAC לתמותה ואירועים קרדיווסקולרים, לתמותה כללית ולהערכת סיכון אינדיווידואלית במניעה ראשונית (מבואות 30-74)

יתרונו הבולט של CCS על פני גורמי הסיכון הקלסיים והחדשים בחיזוי אירועים ותמותה נובע מהיותו מדד למחלה הטרשתית עצמה ולכמותה המשקפת את מכלול גורמי הסיכון המאיצים את התהליך. הכללת ה CCS בהערכת הסיכון האינדיבידואלי לאירוע או תמותה קרדיאלים מהווה תוספת משמעותית ואמינה בחיזוי האירועים. פורסמו עבודות רבות שהצביעו על ערכו המוסף של ה CCS בשינוי חיזוי רמת הסיכון באוכלוסיות שונות לקטגוריית סיכון נמוכה או גבוהה יותר מכפי שהוגדרה בהערכת הסיכון הקלינית דוגמת פרמינגהם, הסקור (Score) האירופאי או על פי סרגל החישוב של ה AHA מ-2013 המבוסס על ה-POOLED COHORT EQUATION (PCE) RISK ‏(10-11) המהווה גם בסיס להנחיות האמריקאיות החדשות. במחקרים רבים שבוצעו באוכלוסיות נבדקים אי-תסמיניים הוכחה עליונותו הפרוגנוסטית של ה CCS בנשים וגברים, בצעירים ומבוגרים ובקבוצות סיכון גבוהות, דוגמת סכרתיים, היפרטנסיבים, דיסליפמים, מעשנים וחולי אי ספיקת כליות. במחקרים אלה הוכח כי הוספת ה CCS להערכת הסיכון הקליני מביאה לרקטגוריזציה של עד מחצית הנבדקים בעלי סיכון פרמינגהם בינוני. קבוצת חוקרי מחקר ה -(Coronary Artery Risk Development in Young Adults (CARDIA Study) (45 פרסמה כי גם בצעירים גילאי 32–45, נשים וגברים, נוכחות הסתיידויות מעלה באופן משמעותי את הסיכון לתמותה ולאירועים קרדיו-וסקולרים במעקב של 15 שנים. בצעירים בגילאים אלה שכיחות טרשת מסוידת נמוכה ביותר, מאחר שמרבית הפלאקים הטרשתיים בשלבי הטרשת הראשוניים אינם מסוידים. טרשת זו ניתנת להדגמה באולטרסאונד (בעיקר תלת-מימד) של עורקי הצוואר ובעורקים הפמורלים. ההנחיות האירופאיות להערכת סיכון קרדיווסקולרי (מאוגוסט 2019) מכלילות טכניקת אולטרסאונד זו כבדיקה לרקטגוריזציה של הסיכון בנוסף ל CCS ברמת המלצה של IIA‏ (2).

ניתן ללמוד על הערך הפרוגנוסטי של ה CAC על הקשר, הדיסקרמינציה, והרקלספיקציה של ה CCS לאירועים קרדיו-וסקולרים )תמותה כלילית ואוטם לא פטאלי (במעקב ממוצע של 5 שנים, בעיקר מתוצאותיהם של שני מחקרים מובילים ה-( THE MULTI- ETHNIC STUDY OF ATHEROSCLEROSIS (MESA בארצות הברית (37) ומחקר ה-Heinz Nixdorf‏ (40) בגרמניה. במחקר ה MESA מוכחת עליונות ה CCS לעומת גורמי סיכון מקובלים בעבודה בה הושוו באופן פרוספקטיבי ערכן הפרוגנוסטי של 6 גורמי סיכון שונים (46). נכללו בעבודה זו 1330 נבדקים ממשתתפי מחקר ה MESA המקורי שהיו כולם בעלי סיכון בינוני לפי מדד ע"ש פרמינגהם, ללא סוכרת וללא מחלה קרדיו-וסקולרית ידועה שבהם נמדדו כל גורמי הסיכון. לאחר מעקב חציוני של 7.6 שנים, נצפה שיפור ב net reclassification לאירועים קרדיאלים גבוה יותר כשהוסף ה CCS למודל החיזוי לעומת הוספת כל אחד מהגורמים האחרים כדלקמן:

פורסמו תוצאות המשך עם 10 שנות מעקב אחר נבדקי מחקר ה MESA‏(38) . תוצאות המחקר חיזקו את התובנות על ערך ה CCS ומצאו שאירועים קרדיו-וסקולרים וקרדיאלים קשורים ישירות לנוכחות CAC וכמותו. בעוד שבנבדקים עם 0=CCS היו פחות מ 5% אירועים ב 10 שנים, באלה עם 100=CCS שכיחות האירועים הייתה מעל 7.5%. נתונים אלה תומכים בהנחיות האמריקאיות החדשות (נובמבר 2018) בטיפול בשומני הדם, לפיהן ניתן להימנע מסטטינים בהיעדר הסתיידויות (פרט לאלו ללא סוכרת, מעשנים, בעלי סיפור משפחתי ענף ובאלו עם 190>LDL) או להמליץ על לקיחתם בכל אלו עם 100<CCS.

קיימת הסכמה ש CCS הוא מדד מומלץ לחיזוי תחלואה ותמותה קרדיו-וסקולרים. הבדיקה מומלצת בעיקר לאלה עם דרגת סיכון בינונית שעפ"י הגדרת ה-PCE SCORE עומדת על 10Year ASCVD Risk של 7.5%-20%, או בכל הנבדקים שקיים לגביהם ספק בצורך בטיפול עם סטטינים למניעה ראשונית, או באלו המתקשים בנטילתם עקב תופעות לוואי.

העדר הסתיידויות - Negative risk marker for CV Disease (מבואות 75-82)

במחקרים בהן נבדקה המשמעות הפרוגנוסטית של CAC במעקב עד 15 שנים, נמצא שלנבדקים ללא הסתיידויות, סיכון נמוך ביותר לאירועים קרדיאלים, קרדיו-וסקולרים לתמותה כללית וקרדיאלית. הפרוגנוזה המצוינת בהיעדר הסתיידויות משותפת למגוון רחב של נבדקים אי-תסמיניים בכל דרגות הסיכון, לרבות חולי סוכרת, דיסליפידמיה ויתר לחץ דם. במחקר ה MESA, נבדקה התועלת של היעדר הסתיידויות בהורדת קטגוריית הסיכון כפי שנקבעה על ידי המדדים הקליניים לפני בדיקת ה CT, וכונתה בפי החוקרים Negative risk marker for CV Disease‏ (78). בעבודה זו נערכה השוואה ל 13 גורמי סיכון קליניים שהוגדרו על סמך היותם בתחום התקין כגורם סיכון "שלילי" (סמן להיעדר אירועים). בהתבסס על תוצאות מעקב ארוך הטווח של מחקר ה-MESA נמצא, שבקבוצת הנבדקים עם 0=CCS היו הכי מעט אירועים, בהשוואה לאחוז האירועים בקבוצות הנורמה כפי שהוגדרה על פי גורמי הסיכון האחרים. בהשוואה לכל גורמי הסיכון השליליים שנבדקו, נמצא כי 0=CCS הוריד את ה- POST TEST RISK כלפי מטה באופן מובהק ביותר לעומת הגורמים האחרים שנבדקו.

מתוך ההנחיות האמריקאיות AHA/ACC לטיפול בכולסטרול מנובמבר 2018‏ (1) מועמדים לביצוע - CCS שלגבם הידיעה כי לא נמצאו הסתיידויות כליליות עשויה להועיל בהחלטה על טיפול או מניעת טיפול בסטטינים :

  • חולים בהם הטיפול בסטטינים אינו חד-משמעי המבקשים להבין ביתר דיוק את התועלת לעומת הסיכון שבטיפול
  • חולים המבקשים לדעת באיזו מידה הם חייבים לחדש סטטינים לאחר שהופסקו עקב תופעות לוואי
  • מבוגרים (גברים 55–80, נשים 60–80) עם פרופיל גורמי סיכון נמוך השואלים אם יפיקו תועלת מטיפול בסטטינים
  • נבדקים בני 40–55 עם סיכון גבולי של 5% עד פחות מ 7.5% לאירוע קרדיו-וסקולרי ב 10 שנים הבאות על פי ה-PCE אך בעלי גורמים מעלי סיכון

במרבית המקרים נבדקים ללא הסתיידויות דורגו מחדש לדרגת סיכון אינדיבידואלית מתחת לסף הצורך בטיפול תרופתי. כן הוכח מתוך מחקר ה-MESA כי מחצית מכלל הנבדקים שנמצאו לפי ה pooled cohort equation (PCE) 10-year risk כמועמדים לטיפול בסטטינים : CONSIDERED = דרגת סיכון 5% - 5 ‏ 7> לפתח אירוע קרדיאלי ב 10 שנים על פי PCE, או כמחויבים בנטילתם RECOMMENDED = סיכון 7.5 אחוזים ומעלה, ידורגו בהיעדר הסתיידות לדרגת סיכון הקטנה מ 5%, שאיננה מומלצת לסטטינים.

ממצאים התומכים בתוצאות אלה פורסמו בעבודה מקוריאה (85) בה עקבו אחר התמותה הכללית ב 31,375 נבדקים אי-תסמיניים במשך 5 שנים (טווח 3 עד 7 שנים) וחקרו את התועלת של ה CCS ברקלסיפיקציה של המועמדים לטיפול בסטטינים על פי הגדרת ה PCE של ה 2013 AHA.

באורח דומה מצאו חוקרי ה MESA כי כמחצית מאוכלוסיית המומלצים על פי מחקר ה-JUPITER לטיפול ברוזובאסטטין במינון גבוה על סמך ערכי (C-reactive protein (CRP מעל 2 מ"ג/ליטר, היו ללא הסתיידויות ותוייגו לקטגוריית סיכון נמוכה שאינה מחייבת טיפול כלל: על פי מחקר זה ה-5 YEARS NNT למניעת אירוע כלילי היה 24 באלו עם 100<CCS לעומת 549 באלו עם 0=TCS‏ (42).

במטה-אנליזה שכללה מעל 70000 נבדקים נמצא, כי ל-29000 (41%) היה 0=CCS וכי במהלך 4.3 שנות מעקב נצפה שיעור נמוך ביותר של אירועים קרדיו-וסקולרים שעמד על 0.5% לאורך כל התקופה.

תמיכה לערכן הקליני של היעדר הסתיידויות גם באוכלוסיית מבוגרים יותר, ניתן למצוא בעבודה שפורסמה מתוך מחקר ה-BIOIMAGE‏ (86) שעקב באופן פרוספקטיבי אחר 5805 נבדקים בגיל ממוצע של 69 שנים לאחר שנאספו 13 גורמי סיכון ביולוגים וכן ה-CCS‏ (75). תוצאות המעקב הראו כי 0=CCS וכמות מינימלית של הסתיידות של 10>CCS הכתיבו סיכון נמוך ביותר לאירוע קרדיאלי או קרדיו-וסקולרי גם באוכלסית מבוגרים. המחברים ומאמר המערכת מדגישים את החשיבות הקלינית של העדר הטרשת כסמן לפרוגנוזה מצוינת תוך הטבעת המושג של "NEGATIVE RISK FACTOR". העדר הסתיידויות במבוגרים מעל גיל 75 מעמידה בספק את הצורך לסטטינים במניעה ראשונית.

חוסר ההתאמה הנרחב בין דרגות הסיכון הקליניות וההמלצות המקובלות לטיפול בסטטינים לבין הסיכון האינדיבידואלי המבוסס על שילוב ה-CCS עם גורמי הסיכון המקובלים, הוביל את חוקרי ה-MESA לפתח סקור חדש ה- MESA RISK SCORE לחישוב הסיכון הקרדיאלי ל 10 שנים (52). סיקור זה מראה באחוזים את הסיכון לאירוע קרדיאלי ב 10 שנים הבאות על סמך גורמי הסיכון של החולה בלבד ואחר כך מחשב את הסיכון עם שקלול ה CCS.

MESA RISK SCORE

  • מהווה כלי עזר חשוב הן לרופא המטפל והן לחולה בדיון המשותף על הצורך ומידת האינטנסיביות בנטילת סטטינים ואספירין למניעה ראשונית
  • סרגל סיכון זה הוא הראשון והיחיד המכליל כימות טרשת כלילית בחישוב הסיכון הקרדיאלי. הוא מיועד לנשים וגברים אי-תסמינים גילאי 45–85 ומותאם לדרגות הסיכון של המלצות ה 2013 AHA לטיפול בסטטינים באופן שניתן ליישם את תוצאותיו לדרגות סיכון אלה
  • רלוונטי גם בחולים בסיכון גבוה דוגמת חולי סוכרת מעשנים והיפרטנסיבים

להיעדר הסתיידויות (0=CCS) חשיבות בהחלטה על טיפול תרופתי בסטטינים למניעה ראשונית. במספר עבודות הוכח כי הסטטינים אינם מפחיתים אירועים באלה ללא הסתיידויות כליליות. על פי ההנחיות האמריקאיות מנובמבר 2018 ניתן להימנע מטיפול בסטטינים או הפסקתם במניעה ראשונית בנבדקים גילאי 45–75 עם רמות LDL של 70–189 מ"ג/ד"ל אם אינם חולי סוכרת, אינם מעשנים וללא סיפור משפחתי כבד של מחלה כלילית מוקדמת. במקרים אלו ההמלצה היא לחזור על הבדיקה לאחר 3 שנים.

תפקיד ה-CAC ב-SHARED DECISION MAKING (מבואות 83-93)

באוגוסט 2019 פרסם האיגוד הקרדיולוגי האירופאי (2) את המלצותיו לשימוש בדימות הטרשת להערכת הסיכון לאירוע קרדיו-וסקולרי כדלהלן:

Detection of CAC with non-contrast CT gives a good estimate of the atherosclerotic burden and strongly associate with CV events.

Recommendation for CV imaging for risk assessment of ACVD risk

CAC score assessment with CT should be considered as a risk modifier_in the CV risk assessment of asymptomatic individuals at low or moderate risk (CLASS IIa, level of evidence B).

הודות למידע הרב על ערכו הפרוגנוסטי של ה CCS בהערכת הסיכון האינדיבידואלי לאירוע קרדיו-וסקולרי בנבדקים אי-תסמינים, ניתן לראות בו כלי עזר חשוב המסייע לדון בסוגיה השכיחה של נטילת סטטינים למניעה ראשונית. בעבודה שפורסמה נבדקה יעילות הסטטינים במניעת אירועים קרדיו-וסקולרים ב-13,664 נבדקים שעברו CCS עם מעקב של 10 שנים אחר התפתחות MACE. נמצא כי הסטטינים הועילו רק באלה עם CAC ובתלוי בכמותו עם NNT למניעת אירוע MACE ראשוני אחד עמד על 100 בנבדקים עם -1 100=CCS ויורד ל-12 בנבדקים עם 100<CAC בעוד באלה ללא הסתיידויות לא נמצאה תועלת בסטטינים בהורדת האירועים.

ממצאים אלו מאפשרים להציע בדיקת CCS בנבדקים הנמצאים בקבוצת ה-BENEFIT GROUP הרביעית של ה-2013 AHA:

“individuals without clinical ASCVD or diabetes who are 40 to 75 years of age with LDL 70-189 mg/dl and an estimated 10-year ASCVD risk of 7.5% or higher”.

כשליש מנבדקים אלה המוגדרים כעת כ STATIN INDICATED יעברו דרוג מחדש עקב היעדר הסתיידויות לקטגורית סיכון נמוכה של פחות מ 5% לאירוע קרדיו-וסקולרי ב 10 שנים הבאות, בה אין כל מידע המוכיח את תועלת הסטטינים. שיתוף הנבדק במידע זה תורם להחלטה מושכלת לפני נטילת סטטינים למניעה ראשונית לפי המלצות AHA/ACC מנובמבר 2018.

השיטות לכימות CAC והיבטים רדיולוגים (מבואות 94–120 ,14)

השיטה המומלצת לחישוב ה CCS היא מדד ה-AGATSTON . מדד זה מגדיר הסתיידות כאזור בעל צפיפות CT של מעל 130 HU. שטח הרובד המסויד נמדד במ"מ ומוכפל בפקטור צפיפות על פי מפתח שניבחר אקראית בעת תיאור השיטה:

130-199 HU: 1 200-299 HU: 2 300-399 HU: 3 400+ HU: 4

מכפלת פקטור הצפיפות בשטח הלזיה המסוידת מהווה את הסקור של הלזיה, סכום סקור הלזיות מהווה את הסקור של הכלי (העורק השמאלי, העוקף והימני כשענפיהם הצדדיים נכללים עימם). המדד הסופי הוא סכום הלזיות כולן ונקרא TOTAL CALCIUM SCORE.

מדידה זו דורשת תוכנה ייעודית שניתן לרוכשה ולשותלה בכל מכשירי ה CT בארץ. קריאת הבדיקה אינה דורשת מיומנות רבה, אך נדרש ניסיון שיאפשר להבדיל בין הסתיידות כלילית לבין ארטפקטים או הסתיידויות חוץ כליליות, דוגמת הסתיידות טבעת המסתם המיטרלי המפריעה מאוד בקביעת מדד העורק העוקף, הסתיידויות בפריקרד, או הסתיידויות בדופן האאורטה במוצא העורקים הכליליים. מדידת ההסתיידות נעשית באורח אוטומטי על ידי סימון הפלאק המסויד. מקובלת החלוקה הבאה:

  • no evidence of CAC: calcium score= 0
  • minimal CAC: 1-10
  • mild CAC: 11-100
  • moderate CAC: 101-400
  • severe CAC : >400

רצוי לבצע את הבדיקה תוך סנכרון לאק"ג, (AXIAL, עובי פרוסה 2.5 מ"מ, 340 mA ,120 Kv) וניתן להסתפק במינון קרינה נמוך לאחר שהוכח כי מושגת תוצאה דומה לסריקה במינון הגבוה כפי שבוצעה בעבר.

קיימות שיטת נוספות לקביעת CCS אך על פי ההנחיות העדכניות מטעם ה NORTH AMERICAN SOCIETY OF CT ה CLASS I INDICATION ההמלצה לבצעה בשיטת אגטסטון מאחר שזוהי השיטה היחידה המאפשרת הערכת סיכון על פי ה MESA RISK. פורסמו הנחיות לרדיולוגים המורות לדווח על נוכחות וכימות ה CAC בכל בדיקת CHEST CT הנעשית מסיבות לא קרדיאליות. על פי הנחיות אלה ידווחו הרדיולוגים על ה CAC כפי שהם מדווחים על כל ממצא אקראי, אך אין הם מציעים שהרדיולוג יכנס לדיון עם הנבדק או רופאו על משמעות ה CAC. דיון זה באחריות רופא המשפחה או הקרדיולוג, דבר המחייב מהם להכיר את המשמעות הקלינית של מציאת ה CAC וכמותו.

כמות הקרינה בבדיקה (מבואות 121-124)

מכיוון שבדיקת ה CCS כרוכה בהקרנת נבדקים אי-תסמיניים, הוקדשה תשומת לב רבה להערכת הסכנה הכרוכה בה. מודעות זו הניבה פיתוח שיטות ופרוטוקולים להקטנת כמות הקרינה. הפרוטוקול המומלץ הוא שימוש ב-kVp120, עובי פרוסה של 2.5 מ"מ, צמצום שדה הסריקה לאזור הלב בלבד שימוש ב-ECG GATING והתאמת ה mAs למבנה הגוף, עם אפשרות להורדה עד mAs‏50. כתוצאה מכך הורדה כמות הקרינה לפחות מ-1 mSv, בהשוואה - קרינת הרקע השנתית אליה אנו חשופים ללא קרינה מבדיקות רפואיות היא כ-3mSv. החשיפה לקרינת הכרוכה במיפוי או צינתור כלילי גבוהה מחשיפה זו. דרגת קרינה זו מקובלת על מומחי החברה הצפון אמריקאית של CT והם מוצאים כי התועלת בבדיקה רבה מניזקי קרינה אפשריים.

ניתן לאבחן הסתיידויות כליליות ולהעריך את כמותן מכל בדיקת CHEST CT הנעשה ללא הזרקת חומר ניגוד המתבצעת לשאלות רפואיות שונות. על הרופא המפנה לבקש בהפניה לבדיקה התייחסות וכימות ההסתיידויות.

  • במרכזים בהן קיימת תכנה ייעודית עדיף לחשב את ה CCS על פי AGATSTON (הכרחי להערכת סיכון על פי ה MESA SCORE)
  • איגודי הרדיולוגים בארצות הברית (5) ממליצים לדווח על CAC בכל בדיקת CHEST CT כחלק אינטגרלי בפענוחה. החשיבות הקלינית לכך היא רק באלו בהם הערכת סיכון קרדיווסקולרי תתרום למניעה ראשונית
  • ניתן להעריך קיום וחומרת הסתיידויות בכל מכשיר CT עם או ללא ECG GATED

CCS לאבחון מחלה כלילית חסימתית (מבואות 125-137)

העדר הסתיידויות כליליות בנבדקים אי-תסמינים מעל גיל 50 שנה שוללות בסבירות גבוהה מחלה חסימתית כרונית. כמות הסתיידות מרובה שכיחה בחולים עם מחלה כלילית חסימתית כרונית אך קיימת גם בנבדקים אי-תסמינים בהם הפלאקים המסוידים אינם מפריעים לזרימת הדם עקב תופעת הרה-מודלינג. בנבדקים עם ערכי CCS גבוהים מאוד (מעל 1000) מומלץ לבצע אקו לב במאמץ או מיפוי בהתאמה לנתונים הקליניים. בנבדקים עם מיפוי לב, אקו לב במאמץ או ארגומטריה בלתי החלטיים וכן באלה עם כאבי חזה ללא מאפייני תעוקה טיפוסית, תומכת כמות הסתיידות מרובה בקיום מחלה חסימתית ומסייעת בהחלטה על צינתור, בעוד היעדר הסתיידויות תומכת במבחני מאמץ או מיפוי חיוביים שגויים ובכאבי חזה כרוניים שלא ממקור כלילי. יש להדגיש כי בכ-10 אחוזים עד 20 אחוזים מהלוקים באוטם ראשון לא נמצאו הסתיידויות כביטוי לקרע של פלאק טרשתי בלתי מסויד הגורם לתסמונת כלילית חדה. תובנות אלה מהוות את הבסיס למכשירי דימות היברידים המאפשרים מיפוי וכימות CAC וניתן להיעזר בהם בנבדקים עם חשד למחלה כלילית ללא CAD מאובחן.

CCS אינה בדיקת הבחירה המומלצת לאבחון מחלה חסימתית. לאבחון חסימות וחומרתן מומלץ לבצע CORONARY CTA על פי ההסתמנות הקלינית.

התקדמות CAC (מבואות 138-145)

בניגוד למדדים ביולוגים רבים מדד ה CCS הוא משתנה יציב ואינו משתנה בטווח של ימים עד שבועות כמו מדדי סיכון ביולוגים רבים דוגמת ה-CRP. המלצת ה-AHA/ACC לחזור על הבדיקה כעבור 3 שנים באלה שלא יטופלו בסטטינים ולאחר 5 שנים באלו תחת טיפול. בעבודות השונות נמדדה ההתקדמות לאחר שנה שנתיים או שלוש שנים. מדידה אמינה של התקדמות ה CAC מחייבת חומרות ותכנות התומכות בכימות מדויק המאפשר reproducibility של המדידה. בנוסף ל-CT נעשה שימוש באולטרסאונד תוך קורונרי (IVUS) דו ותלת ממדי לבדיקת השפעת הסטטינים על הפלאק הטרשתי בדופן העורקים הכליליים. הנקודות הבאות מסכמות את הידע שנרכש בנושא זה:

  • התקדמות ה CAC מהירה יותר בחולים עם מחלה כלילית קלינית מאשר באלו ללא CAD
  • קצב התקדמות מהיר הוא סמן למחלה כלילית פעילה וקשור לתמותה ותחלואה קרדיווסקולרית גבוהה יותר מאשר באלה עם קצב התקדמות איטית
  • התקדמות ההסתיידויות הכליליות נעשות על ידי יצירת פלאקים מסוידים חדשים, גדילת נפח ושטח של פלאקים קיימים והתכנסות של מספר מוקדים מסוידים לפלאק אחד

בדיקות CT עוקבות להתקדמות ה CAC, אינן מומלצות כבדיקה קלינית ומשמשות בעיקר למעקב מחקרי.

המלצות נייר העמדה לביצוע CCS
  • נייר העמדה מאמץ את ההמלצות לביצוע בדיקת ה-CCS המובאות בהנחיות לטיפול בכולסטרול של ה-2018 AHA/ACC, נייר החתום על ידי 12 איגודים רפואיים של צפון אמריקה וכן את ההמלצות העדכניות של ה-ESC מאוגוסט 2019
  • הבדיקה מומלצת לשם קביעת סיכון אינדיבידואלי לאירוע קרדיאלי ב-10 שנים הבאות בנבדקים אי-תסמיניים בעלי סיכון קרדיווסקולרי גבולי או בינוני שאינם סובלים ממחלה כלילית, PVD, או לאחר אירוע מוחי
  • ניתן ונכון לבצע הערכת CAC במסגרת הדיון על טיפול בסטטינים למניעה ראשונית בנבדקים אי-תסמיניים גילאי 40–75 בעלי סיכון קרדיווסקולרי של 7.5 אחוזים עד 20 אחוזים ב-10 שנים הבאות ובאופן סלקטיבי יותר באלו בעלי סיכון גבולי של 5 עד 7.5 אחוזים. כשליש עד מחצית מהנבדקים יעברו הגדרה מחדש של הסיכון העשויה לעזור בהחלטה על התחלה, הפסקה או אינטנסיביות של הטיפול בסטטינים במקרים בהם ההתוויה איננה חד-משמעית
  • בהיעדר עבודות פרוספקטיביות המבוססות על דימות הטרשת לקביעת הטיפול ויעדיו, (עבודה רבת משתתפים כזו החלה בהולנד(146). ‏CCS מומלץ בעיקר ככלי עזר בהחלטה על התחלת טיפול בסטטינים או הימנעות מהם, ועל דרגת האינטנסיביות הנדרשת. ההמלצות על יעדי הטיפול על פי תוצאות ה-CCS היו בעבר בדרגת EXPERT CONSENSUS בלבד והפכו להמלצות מקובלות ב-AHA/ACC 2018 וה-2019 ESC כ-CLASS II a
  • בחולים הנוטלים סטטינים עם תופעות לוואי או אלה המסרבים נטילתו: CCS עשוי לסייע בהחלטה שאין הכרח לנטילתם במקרה של 0=CCS . העדר הסתיידויות מוריד, במרבית המקרים את דרגת הסיכון לאירוע קרדיו-וסקולרי לפחות מ 5% בה אין כל הוכחה ליעילות הסטטינים במניעת אירועים עתידיים. מאידך, כמות הסתיידות מרובה (100<CCS) תעזור בהחלטה על טיפול אנטי-היפרליפמי חלופי ותעזור בשכנוע הניבדק בצורך לנטילתם
  • בחולים סוכרתיים, מעשנים, עם סיפור משפחתי כבד של מחלה כלילית משפחתית מוקדמת, ובאלה עם רמת LDL מעל 190 מג/ד"ל יש לטפל בסטטינים מבלי להתחשב בדרגת הסיכון המחושבת ואף על פי שהיעדר הסתיידויות מציבה אותם בקטגוריית סיכון נמוכה
  • בחולים היפרטנסיבים קביעת הסיכון האינדיבידואלי על פי ה-MESA SCORE מקנה כלי נוסף העשוי לסייע לרופא המטפל בהתאמה האישית של אינטנסיביות הטיפול או בהחלטה על התחלתו במקרים גבוליים או ב-PRE HYPERTENSION. גישה זו אומצה בהנחיות הישראליות לאבחון וטיפול ביל"ד מטעם ההנחיות העדכניות המשותפות לחברה ליל"ד, האיגוד הקרדיולוגי, רופאי המשפחה והרפואה הפנימית
  • בגברים ונשים מעל גיל 55 מעשנים בהווה או שהפסיקו לעשן בטווח 15 שנים החולפות והעומדים בקריטריונים לביצוע CT חזה במינון קרינה נמוך לצורך גילוי מוקדם של סרטן ראות, מומלץ להעריך את ה-CCS מתוך אותה הסריקה. הרופא המטפל יוכל לקבוע סיכון קרדיאלי אינדיבידואלי (MESA RISK SCORE) ולהיעזר בתוצאות בדיון עם החולה על טיפול תרופתי למניעה ראשונית. הנחיות ה-2018 AHA/ACC מחייבות מתן סטטינים לכל המעשנים
  • CCS איננו מומלץ כבדיקה לברור כאבי חזה ויש להעדיף CORONARY CTA. בבירור כאבי חזה בחולים ללא מחלה כלילית ידועה שמניעה ראשונית רלוונטית לגביהם, ניתן להיעזר בטכניקה היברידית, באם זמינה, המאפשרת גם הערכת CCS בעת מיפוי הלב
  • העדר הסתיידויות הוא הסמן הפרוגנוסטי החזק ביותר להיעדר אירועים קרדיאלים עתידיים אך אינו שולל קיום טרשת בלתי מסוידת. סוג זה של טרשת שכיח יותר בצעירים, בנשים ובאלה עם תסמונת כלילית חדה. הבדיקה איננה מומלצת לכן לברור כאבי חזה אקוטיים (דוגמת חדר המיון)
  • בצעירים בגילאי 30–45 נוכחות הסתיידות איננה שכיחה (10-20% בלבד) אך מציאותה בכל כמות שהיא מכתיבה סיכון מוגבר לאירוע קרדיווסקולרי בטווח הארוך (מחקר ה CARDIA). בקבוצת גילאים זו מומלץ לבצע אולטרסאונד של עורקי הצוואר ו/או הפמורלים לאבחון טרשת סיסטמית המקדימה בעורקים אלו את הטרשת הכלילית המסוידת
  • הפניה לביצוע הבדיקה נתונה לשיקול הרופא המטפל/הקרדיולוג ומחייבת להכיר את יתרונותיה ואת השימוש המושכל בתוצאותיה
  • אין אינדיקציה קלינית לבדיקות CCS שנתיות. בהיעדר הסתיידויות ניתן לשקול ביצוע חוזר לאחר 3 עד 5 שנים באלו שלא יטופלו בסטטינים ולאחר 5–10 שנים באלו תחת טיפול
  • נייר עמדה זה מעודד את הרדיולוגים המפענחים CT חזה לדווח על הימצאות ובמידת האפשר על כמות ההסתיידויות הכליליות
  • נייר העמדה ממליץ בפני כל הרופאים המפנים חולים ל CT חזה ללא הזרקת חומר ניגודי ושמניעה ראשונית קרדיווסקולרית רלוונטית לגביהם, לציין בהפניה בקשה לדיווח על הסתיידויות כליליות וכימותן אם ניתן
